Maybe something similar to "CREATE EXTENSION ... FROM unpackaged"?

Something like:
CREATE EXTENSION myfoo; /* shell extension */
ALTER EXTENSION myfoo ADD type ...;
ALTER EXTENSION myfoo ADD function ...;
...
