This uses the new OUTDATED option for REINDEX.  If user asks for multiple job,
the list of tables to process will be sorted by the total size of underlying
indexes that have outdated dependency.

Cover scram_iterations, which were missed in 16.

Also turn the list into a <simplelist> with 2 columns, which is much
nicer to read.
