Allow passing a pointer to GetNamedDSMSegment()'s init callback.

This commit adds a new "void *arg" parameter to
GetNamedDSMSegment() that is passed to the initialization callback
function.  This is useful for reusing an initialization callback
function for multiple DSM segments.
