Try adding the g flag to the regex (for global). From the documentation:
regexp_replace('foobarbaz', 'b..', 'X')
fooXbaz
regexp_replace('foobarbaz', 'b..', 'X', 'g')
fooXX
regexp_replace('foobarbaz', 'b(..)', E'X\\1Y', 'g')
fooXarYXazY
